Service Description
A thorough evaluation, explanation of findings, and initiation of treatment. Health insurances accepted: Aetna, UHC, BCBS / Anthem, Meritian Health, Medishare, and UMR. In network eligibility to be determined. It is recommended to call your insurance to validate benefits. Abundant Physical Therapy + Wellness does not participate with Medicare or Medicaid for Physical Therapy. If you are a Medicare or Medicaid patient, please book at a participating facility. Medicare patients are welcome to book Wellness visits.
Cancellation Policy
New address: 2500 Youngfield Street, Suite 5, Lakewood, CO 80215. To keep the office secured, there is a key code to enter the front door. The access code is 1876. Please remember to lock the door behind you. If you do not show up for your appointment, or cancel within 24 hours, you will be responsible to pay $50. Abundant Physical Therapy + Wellness reserves the right to discharge patients at any time.
