*Individual plans will be verified prior to the first appointment to determine in network eligibility. Please allow 3-5 business days if you are new to the practice to have your information verified. 

Abundant Physical Therapy + Wellness does not participate with Medicare or Medicaid for Physical Therapy.  If you are a Medicare or Medicaid patient, please book at a participating facility.  Medicare patients are welcome to book Wellness visits.
